HEALTH INSURANCE MARKETPLACE NAVIGATOR

Position Description

The HIM Navigator position for Appalachian Mountain Project Access will perform responsibilities related to the primary function of the organization-- providing meaningful access to healthcare. The HIM Navigator will offer in-person application assistance to those seeking health insurance through the health insurance marketplace within an 8-county region. To include private health insurance plans and TN Medicaid. The HIM Navigator will report to the Director of Programs.

 

Job Duties include

  • Provide direct assistance to individuals seeking insurance through the Health Insurance Marketplace
  • Give unbiased insight to individuals on health insurance options to aid them in making insurance selections
  • Complete all Federally required training, Lead Grantee training, Subcontractor/Agency training, and any supplemental training as assigned.
  • Submit to a State directed background check and fingerprinting.
  • Be willing to travel to outlying rural areas as needed as well as travel to Nashville approximately 2 times a year for Grant Partner Training.
  • Participate in bi-weekly Grant Partner conference calls, attend webinars, keep up with assigned equipment, and complete required paperwork
  • Work with TennCare presumptive and other eligible individuals at local Health Departments
  • Attend community meetings and events as assigned
  • Host educational booths and enrollment events as assigned by Lead HIM Navigator
  • Assists with other Activities as assigned by Directors

 

Job Expectations include

The HIM Navigator will be able to navigate Microsoft Office products such as Word, Excel, Power Point; an email account and shared calendar; operate office equipment such as a copier/printer/scanner, postage machine, etc. The individual will be able to make appropriate and competent detailed notes into a database system. The individual will be professional as well as get along with a small office staff. The individual will collaborate and communicate with patients, providers, community members, and other AMPA staff with respect, empathy, genuineness, and specificity. The individual will maintain confidentiality of all patient data and organization operations/strategic planning. The individual will adhere to a Code of Ethics and perform in keeping with the organization's Policies & Procedures, Employee Handbook, and all applicable laws/regulations. The individual will either have or gain knowledge regarding the Affordable Care Act, the Health Insurance Marketplace, Medicaid/Medicare, and other health insurance options. The individual will maintain empathy while establishing and preserving appropriate boundaries. The HIM Navigator will be willing to learn, teach, and advocate for the organization's mission, vision, and various program aims. The individual will be punctual, dependable, flexible and able to contribute to tasks and projects that may not necessarily fit within the above job description. The individual will be willing and able to assist in growth of the organization.

Job Qualifications include

COMMUNITY NAVIGATOR

Position Description

The Community Navigator position for Appalachian Mountain Project Access will perform responsibilities related to the primary function of the organization-- providing meaningful access to healthcare. The Community Navigator will serve the organization in the Care Coordination Program by meeting with patients to determine program eligibility. The Community Navigator may be placed in a clinic, hospital, or AMPA office.

Job Duties include

  • Complete AHCN screener with patients to determine referral and social care management plan
  • Helps educate patients, providers, and the community on the services of the organization
  • Works to identify patient social needs and helps coordinate connection to these resources as appropriate
  • Prepare a mutually developed Resource Plan
  • Follow-up with Patients to determine accessibility of Resources
  • Receives and returns phone calls and requests from patients in a prompt and courteous manner
  • Effectively terminate relationships with patients
  • Partner with members of the patient’s AHCN team
  • Explore social and community resources
  • Participate in community committees and resource fairs, collaborative projects, education initiatives, and presentations as needed
  • Maintains patient and physician files in an accurate and organized manner, while ensuring confidentiality
  • Attend staff and departmental meetings for AMPA and assigned location as required
  • Appropriately document all encounters with patients and providers in database
  • Participate in office projects/team assignments
